On August 18, 2026, the FDA opened a public discussion about how generative-AI medical devices should be evaluated before launch and watched after launch. The agency specifically raised questions about risk, premarket testing, postmarket monitoring, foundation models, and agentic AI. The paper is not final guidance. It is an early request for feedback. Framingham made repeated observations useful by comparing the same people over time.
In 1948, researchers started the Framingham Heart Study in Framingham, Massachusetts. They recruited 5,209 men and women from the town. The idea was unusual for its time: do not wait for one dramatic event. Follow the same people again and again.
Participants returned for repeated exams. Researchers recorded things such as blood pressure, blood chemistry, smoking habits, medical history, and other health measures. Over time, those repeated records helped scientists see links that one office visit could miss.
Today, words like “risk factor” sound normal. Framingham helped make that way of thinking common. High blood pressure, smoking, high cholesterol, obesity, diabetes, and inactivity became easier to study because researchers could compare a person’s later health with what had been recorded earlier.
The key was not one perfect measurement. It was the baseline plus the next measurement, then the next one after that. A number gained meaning because there was something to compare it with.
Your seven-day note is not a medical study, and it cannot diagnose anything. The scale is completely different. But the narrow lesson travels well: memory is weak at comparing ordinary days. A small record can show what “normal for me” looked like before a new routine, a bad week, or a changing device score.
That is why a baseline matters. It does not tell you what a symptom means. It gives you a clearer starting point for noticing change and asking better questions.

Ancient Greek medicine also carried beliefs that modern science has rejected. We should not copy the treatments or assume those writers understood disease the way medicine does today.
But one useful habit survived: write down what you actually observed before building a story around it. That protects you from a very human mistake. We remember the worst night. We forget the five ordinary nights that came before it.
Modern devices can collect far more information than an ancient physician could imagine. The narrow comparison is about the human layer beside the machine layer. “Slept six hours. Energy 2 out of 5. Stress 4 out of 5.” That note is crude, but it is easy to understand later.
A notebook does not replace a clinician, a test, or a medical device. It gives you one stable reference point when your routine changes or when the software interpreting your data changes.
